INITIAL PSYCHIATRIC EVALUATION
An initial psychiatric evaluation is an invaluable and necessary first step in the assessment, diagnosis, and treatment of conditions impacting mood, memory, thought processes and behaviors. The evaluation allows your provider to develop a clear and thorough understanding of your history and current concerns, which leads to accurate diagnosis, evidence-based treatment, and the establishment of an effective & long-term therapeutic relationship.
Appointment Duration: 60 minutes
Available for individuals 6 years of age and older
PSYCHIATRIC FOLLOW UP & MEDICATION MANAGEMENT
It’s important to note that not all individuals and psychiatric concerns will require medication. However, there are many situations in which prescription medications, in addition to interdisciplinary psychiatric care, are an effective tool in symptom relief, maladaptive behavior reduction, and improved quality of life.
Current research concludes that evidence-based medication recommendations can be effective in treating:
ADHD (both stimulant and non-stimulant options available)
Anxiety, Generalized Anxiety Disorder & Phobias
Depression
Bipolar Disorder, Types I and II
Eating Disorders
Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, ARFID, and Atypical Eating Disorders
Obsessive-Compulsive Disorder (OCD)
Post Traumatic Stress Disorder (PTSD)
Schizophrenia
Sleep Problems
Addiction
Substance abuse
Appointment Duration: 30 minutes
Available for established patients only.
Disclaimer: prescriptions (& refills) will only be written for patients who maintain their regularly scheduled appointments and are in good standing with Healthy Minds Behavioral Health and the prescriber. Prescriptions are written at each appointment. If you need a refill in between appointments, a prescription may be written to cover you until your next scheduled appointment, at the prescriber’s discretion. Please note it may take up to 72 hours (excluding weekends/holidays/vacations) for the prescription to be submitted after your request is received.
Controlled Substance Prescription Laws:
Controlled substances can only be written for 30 days at a time. This includes, but is not limited to: Adderall, Vyvanse, Focalin, Xanax, Ativan, & Ambien. The law also requires that a new prescription be written each time these medications are filled, and no exceptions can be made by your prescriber OR pharmacist for early refills or for refills >30 days. Please make sure to keep your medications locked up and in a safe place to prevent loss and theft.

BILLING & PAYMENTS:
Payment is due in full at the time of service, unless you are covered by one of the carriers contracted. It is recommended that you contact your insurance company to confirm your coverage for services provided by Healthy Minds Behavioral Health. Insurance copays, deductibles, and non-covered expenses are due on the day of the appointment. You will be asked to pay any outstanding balances prior to being seen, unless an alternative financial arrangement has been made. By proceeding with services, you acknowledge that you will be financially responsible for any remaining balance not covered by your insurance.
